An exciting opportunity has arisen to join our clients friendly team based in Lymington.
You will be responsible for supporting the Office Team to ensure that the care delivered is of the highest standard.
Job Description
The role is to support the delivery of care services by carrying out a range of functions in the field, including the assessment and review of individual client services and the practical supervision, instruction and support of the carers.
The key aspects of the role will include:
- Providing full support to the team of Care Givers.
- Working closely with the Care Manager to co-ordinating care services and ensuring the perfect Care Giver and client match.
- Liaising with family members and relevant healthcare professionals in relation to client care.
- Assisting in the planning, co-ordinating, and scheduling of client support, ensuring continuity of service at all times.
- Assisting with Care Giver training activities.
- Managing daily medication and task prompts.
- Following up alerts to ensure a timely service.
- Managing and recording Care Giver annual leave requests.
- Assisting with Care Giver recruitment.
- Dealing promptly and appropriately with any queries received, referring to the relevant person or team.
- Building and maintain effective communication with all clients, relatives, Care Professionals and associated professionals.
- Writing care plans for new clients.
- Supporting local community and business events and activities.
- Ensuring compliance and quality assurance standards are met in line with company procedures and CQC regulations.
- Reporting any recruitment requirements to ensure client needs are fulfilled.
The Care Co-Ordinator will split their working time equally between community office and field based activities.
